Download the appropriate PDF from the Paper Forms Library. We strongly encourage you to utilize online filing whenever possible. If you are using a paper form, we recommend that you submit your paper filing through our digital mail feature online.
You can also mail or hand-deliver your forms. This is strongly discouraged as it is the slowest option, requires a check or money order (no credit cards), and expedited service is not available.
